Few in U.S. see doctor or get medication for flu: study

Only a small percentage of people who get influenza or a similar illness are ever prescribed drugs shown to help the virus, according to a study released on Monday.

The survey of flu patients also showed that about five percent of U.S. children see a doctor or nurse for influenza-like illness, compared to just about 2 percent of adults.

Thomson Reuters Healthcare surveyed insurance claims covering nearly 20 million people with health insurance over two flu seasons in 2005-2006 and 2006-2007.

Anywhere between 5 percent and 20 percent of the population gets flu in a given flu season, the CDC estimates.

The researchers, part of one of the research divisions of media and information firm Thomson Reuters, were looking at influenza-like illnesses, which include influenza proper but also other respiratory diseases that cause fever, cough and other symptoms.

They found that 4 to 6 percent of patients with influenza like illnesses filled a prescription for an antiviral medication. Roche and Co’s Tamiflu and GlaxoSmithKline’s Relenza are both recommended for treating influenza.

But most clinics do not use on-the-spot flu tests, and use of the drugs is low.

Doctors usually recommend only rest, fluids and perhaps analgesics for treating viruses such as flu, although influenza kills 36,000 Americans in an average flu season, according to the U.S. Centers for Disease Control and Prevention.

Most of those who die are elderly or have chronic disease, but the Thomson Reuters data showed that patients with flu-like illness who seek medical treatment are more likely to be children.

“Approximately 1 in 18 children, 1 in 38 elderly persons, and 1 in 45 adults used influenza-like-illness-related inpatient or outpatient services in each flu season,” the report reads.

This works out to 5.6 percent of children, compared to 2.6 percent of the elderly.

But older people in general did use doctors more often.

“A greater percentage of elderly enrollees (53.4 percent) than adult (32.4 percent) or child enrollees (26.4 percent) used any inpatient or outpatient health services in flu season 2006-2007,” the report reads.

Cause of cartilage degeneration in osteoarthritis uncovered

Scripps research scientists have made a breakthrough discovery by finding the underlying cause of progressive deterioration of the cartilage, which characterises osteoarthritis- loss of the protein HMGB2.

The researchers showed how the loss of HMGB2, found in the surface layer of joint cartilage, leads to degeneration of cartilage in people suffering affected by osteoarthritis.

“We have found the mechanism that begins to explain how and why aging leads to deterioration of articular cartilage. Our findings demonstrate a direct link between the loss of this protein and osteoarthritis,” said Scripps Research Professor Martin Lotz, M.D., a world-renowned arthritis researcher who led the study with Noboru Taniguchi, M.D., Ph.D., a senior research associate in his lab.

Typically, osteoarthritis begins with a disruption of the surface layer of cartilage, called the superficial zone, which triggers an irreversible process that eventually leads to the loss of underlying layers of cartilage until bone begins to grind painfully against bone.

Osteoarthritis most commonly affects the spine, temporomandibular joints, shoulders, hands, hips and knees.

“We knew that the first phase of osteoarthritis is the destruction of cartilage in the superficial zone. Now we know that before this layer is destroyed, there is loss of the critical DNA binding protein HMGB2 and that this loss is directly related to aging,” said Lotz.

It was found that the protein HMGB2 is uniquely expressed on the surface layer of cartilage in joints, where it supports the survival of chondrocytes, the cells that produce and maintain cartilage.

Aging is associated with the loss of HMGB2 and an accompanying reduction or total elimination of chondrocytes in the superficial zone.

In fact, the scientists provided further links between HMGB2 and osteoarthritis by genetically breeding HMGB2 deficient mice that had an earlier and more severe onset of osteoarthritis.

The findings provide a promising avenue to explore the development of new osteoarthritis treatment options.

“If small molecules can be found to prevent or stop the loss of HMGB2, or conversely, to stimulate the production of this protein, then it is possible that osteoarthritis may one day either be prevented or reversed,” said Lotz.

The finding may provide scientists with a clue about how they might be able to engineer the surface layer cartilage.

“As our population ages, osteoarthritis will become an ever-greater health issue. Everyone eventually gets osteoarthritis; even those people who are not functionally impaired by the disease are found to have cartilage damage. And it all starts with the loss of cells in the superficial layer. We now have a starting point for potential prevention, diagnosis, and treatment,” said Lotz.

The scientists describe their work in this week’’s Early Edition of the Proceedings of the National Academy of Sciences.

Diabetes Epidemic Now Poses Challenges for Nursing Homes

More and more people with diabetes are living to older ages, thanks to medical advances. But the long-term facilities, such as nursing homes, that care for aging Americans may not be ready for the additional challenges that come with treating patients with diabetes.

“We need to spend appropriate time to think of a way to successfully provide care for people with diabetes as they enter their elder years, and we’re just beginning to understand how to do that,” said Dr. Paul Strumph, vice president and chief medical officer for the Juvenile Diabetes Research Foundation.

Although as many as one in four nursing home residents has diabetes, not all are getting care that meets the American Diabetes Association’s goals for community-dwelling adults, according to a recent study.

The study, published in Diabetes Care, found that while 98 percent of nursing home residents with diabetes had their blood glucose levels monitored, only 38 percent met short-term glucose goals.

The better news from the study was that 67 percent of the nursing home residents with diabetes met their long-term glucose control goals, which meant they scored less than 7 percent on their A1C tests. A1C is a measure of long-term blood sugar control.

“One of the key differences in managing diabetes in a nursing home is that it’s often not the condition of primary importance,” said Helaine Resnick, director of research at the Institute for the Future of Aging Services for the American Association of Homes and Services for the Aging.

Resnick said one of the concerns she had with the study findings was that no one has yet to come up with specific guidelines for caring for elderly people with diabetes. Glucose control goals for someone who’s 40 and living at home may well be different than for someone who’s 85, cognitively impaired, and living in a long-term care facility, she said.

Diabetes medications are designed to lower glucose levels, which can prevent complications from developing in diabetic people. But, when you take medicines to lower blood glucose, it can go too low, which can be extremely dangerous, especially for older adults,” Resnick said, noting that it’s difficult to find “the appropriate balance between keeping sugars low with the risk of keeping it too low.”

Strumph pointed out that the needs of older people with diabetes may be different as well. People with type 1 diabetes and people with type 2 diabetes who need insulin often choose to use an insulin pump when they’re younger, but pumps may not be the best choice for someone who’s older and not as aware, he said.

“Someone in a nursing home could pull out a pump site and not know. In that case, you may want to be on a longer-acting insulin instead. We haven’t yet defined what the ideal insulin [regimen] is for someone in a facility with a fairly predictable schedule,” Strumph said.

Both Strumph and Resnick said it’s important for family members, the nursing home resident, and the staff to come up with a realistic care plan.

“Families need to become more actively involved in working with care teams, and that’s true for diabetes and for other conditions. Ensure that the facility understands the family’s and the resident’s preferences. Is your mother more interested in keeping her blood glucose control tight and risk [low blood sugar]? Or is it better for her to ease up on glucose control and work more on quality-of-life issues? Resnick said.

“Families have to be very involved, and the communication needs to be ongoing, because people’s values can change,” she added.

Pfizer drops bid to sell Viagra over the counter in Europe

Pfizer has dropped its bid to market its potency pill Viagra over the counter in Europe, the US pharmaceutical giant announced Thursday.

Pfizer “has withdrawn its application to switch the legal status of the 50 mg tablet strength of Viagra from ‘prescription only’ to ‘non-prescription’ in the European Union (EU),” a statement said.

Prizer said it had decided to withdraw the application “to fully consider” comments from the European Medicines Agencys Committee for Medicinal Products for Human Use, “recognizing that there were some concerns regarding the proposed supply of Viagra 50 mg tablets without a prescription,” the statement said.

“All current doses of Viagra will continue to be available to patients by prescription from their doctor,” it said.

Worldwide sales of Viagra, first introduced ten years ago, reached 1.8 billion dollars in 2007.

Your mobile phone will help you maintain your exercise routine and keep the pounds off over winter months.

Washington University researchers and Intel have created two new cell phone applications, dubbed UbiFit and UbiGreen, to automatically track workouts and green transportation.

In a three-month field experiment, people using UbiFit with the background display kept up their workout routines over the winter holidays, a period when people typically slack off on exercise, while people without the display let their regimen slide.

The programmes display motivational pictures on the phone’s background screen that change the more the user works out or uses eco-friendly means of transportation.

The applications are designed to change people’s behaviour for the better, said Sunny Consolvo, a recently graduated WU Information School doctoral student and one of UbiFit’s creators.

UbiFit and UbiGreen are part of a larger project at the WU to use mobile computing in everyday activities and long-term goals such as fitness, said project leader James Landay, WU computer science and engineering associate professor.

‘You can’t get fit in a short period of time in one place,’ he said. ‘It happens long-term, in many different places and ways.’

Current versions of UbiFit and UbiGreen use an external sensing device (the Intel Mobile Sensing Platform) clipped to the user’s waist. The device includes an accelerometer to sense the user’s movement, according to WU release.

The programmes could run on phones with built-in accelerometers, such as the iPhone and the new Android G1, with no need for external equipment, Landay said. UbiGreen also relies on changing cell phone tower signals to determine whether a person is taking a trip.

The sensing device determines what the user is doing based on how it gets jiggled around, Landay said — the localised motion at your waist will be different if you’re walking, jogging, or sitting in a car.

The sensing device sends signals three times per second via Bluetooth to the cell phone, where the application averages these rapid signals and translates them into, for example, a 20-minute jog or a drive to work.

UbiFit displays an empty lawn at the beginning of the week, and flowers grow as the user works out during the week. Different kinds of workouts yield different coloured flowers.

Users set weekly workout goals and are rewarded with a butterfly when the goal is met. Users can also enter workout information manually if the sensor made a mistake, they forgot to wear it, or they did an activity that the sensor does not detect.

‘The background display was definitely one of the biggest wins of our study,’ Consolvo said.
